Incidencia y factores predictivos de oclusión de la arteria radial tras procedimientos coronarios transradiales

Incidence and predictors of radial artery occlusion following transradial coronary procedures

RESUMEN

Introducción y objetivos: El acceso transradial para procedimientos coronarios percutáneos ha crecido en popularidad debido a sus ventajas sobre el abordaje femoral. Sin embargo, este beneficio se ve ensombrecido por una mayor tasa de oclusión de la arteria radial (OAR). Nuestro objetivo fue evaluar la incidencia y los factores predictivos de OAR tras el cateterismo transradial. También se estudiaron las variaciones anatómicas de la arteria radial (AR).

Métodos: En este estudio prospectivo participaron 427 pacientes a los que se había realizado angiografía coronaria o angioplastia mediante acceso transradial. Se realizó una evaluación ecográfica de las arterias del antebrazo. En caso de OAR, se llevó a cabo otro control ecográfico al mes y a los 3 meses de la intervención.

Resultados: La población de estudio incluyó a 288 varones (67,4%) y 139 mujeres (32,6%). La edad media fue de 61,9 ± 11,1 años. La OAR se produjo en 48 pacientes (11,24%), de los cuales en 15 (32,6%) se produjo recanalización espontánea en el plazo de 3 meses. En el análisis multivariante, la edad más joven (OR = 0,642; IC95%, 0,480-0,858; p = 0,031), la presión arterial sistólica periprocedimiento baja (OR = 0,598; IC95%, 0,415-0,862; p = 0,007), el diámetro radial pequeño (OR = 0,371; IC95%, 0,323-0,618; p = 0,031), la anticoagulación insuficiente (OR = 0,287; IC95%, 0,163-0,505; p < 0,001), la hemostasia oclusiva (OR = 0,128; IC95%, 0,047-0,353; p < 0,001) y la larga duración de la hemostasia aparecieron como predictores independientes de OAR. La incidencia global de variaciones anatómicas de la AR fue del 14,8% (n = 63). Entre estos pacientes, 40 (63,5%) tenían un origen radial alto, 18 (28,6%) presentaban una tortuosidad extrema de la AR y 5 (7,9%) tenían un asa radiocubital completa.

Conclusiones: La heparinización insuficiente y la hemostasia oclusiva son los principales predictores de OAR modificables. La estrategia preventiva debe centrarse principalmente en estos 2 factores predictivos.

ABSTRACT

Introduction and objectives: The use of transradial access for percutaneous coronary procedures has increased due to its advantages over the femoral approach. However, this benefit comes at the expense of a higher rate of radial artery occlusion (RAO). Our objective was to assess the incidence and predictors of RAO following transradial catheterization. Additionally, we studied anatomic variations of the radial artery (RA).

Methods: This prospective study enrolled 427 patients who underwent coronary angiography or angioplasty via transradial access. The forearm arteries were evaluated by ultrasound. If RAO was present, follow-up ultrasound examinations were performed at 1 and 3 months postprocedure.

Results: Our study population included 288 men (67.4%) and 139 women (32.6%). The mean age was 61.9 ± 11.1 years. RAO occurred in 48 patients (11.24%), and spontaneous recanalization was observed within 3 months in 15 patients (32.6%). On multivariate analysis, independent predictors of RAO were younger age (OR, 0.642; 95%CI, 0.480-0.858; p = .031), low periprocedural systolic blood pressure (OR, 0.598; 95%CI, 0.415-0.862; p = .007), a small radial diameter (OR, 0.371; 95%CI, 0.323-0.618; p = .031), insufficient anticoagulation (OR, 0.287; 95%CI, 0.163-0.505; p < .001), occlusive hemostasis (OR, 0.128; 95%CI, 0.047-0.353; p < .001), and long duration of hemostasis. The overall incidence of RA anatomic variations was 14.8% (n = 63). Among these, 40 patients (63.5%) had a high radial origin, 18 (28.6%) had extreme RA tortuosity, and 5 (7.9%) had a complete radioulnar loop.

Conclusions: The main modifiable predictors of RAO are insufficient heparinization and occlusive hemostasis. Preventive strategies should focus primarily on these 2 predictive factors to reduce the risk of RAO.

INTRODUCCIÓN

El uso del abordaje transradial para realizar intervenciones coronarias se ha convertido en una práctica habitual en cardiología intervencionista por las innumerables ventajas que ofrece1, hasta tal punto que las guías de práctica clínica vigentes lo recomiendan como abordaje de primera línea2.

No obstante, los beneficios de esta técnica se ven atenuados por el riesgo de oclusión de la arteria radial (OAR) asociado con tasas que se sitúan entre el 5 y el 30%3,4. El objetivo de este estudio fue valorar mediante ecografía Doppler la incidencia y predictores de OAR tras la realización de cateterismos transradiales.

MÉTODOS

Población de pacientes

Se realizó un estudio longitudinal, prospectivo y unicéntrico en la unidad de cardiología del Hospital Central Militar en Argel (Argelia). Tras aplicar criterios de exclusión (inestabilidad hemodinámica e infarto de miocardio con elevación del segmento ST), se incluyó a un total de 427 pacientes consecutivos tratados de intervenciones coronarias transradiales entre enero de 2019 y marzo de 2020. El estudio se llevó a cabo de conformidad con los principios establecidos en la Declaración de Helsinki y las Buenas Prácticas Clínicas de la Conferencia Internacional de Armonización tras ser aprobado por el comité de ética local. Todos los pacientes dieron su consentimiento informado por escrito.

Canalización de la arteria radial y arteriografía radial mediante abordaje retrógrado

Tras la punción de la arteria radial (AR), se introdujo una vaina hidrofílica radial (Radiofocus II, TERUMO Medical, Japón o Prelude, MERIT Medical, Estados Unidos). Después, a criterio del operador, se administró un cóctel antiespasmódico en la AR a través de la vaina introductora consistente en 1 solución salina, 1 vasodilatador (1 ml de nicardipino) y 1 bolo de heparina no fraccionada por vía IV o directamente en la AR. En aquellos pacientes a tratamiento con antagonistas de la vitamina K, los fármacos no se suspendieron antes de la intervención.

Se realizó una arteriografía radial mediante abordaje retrógrado inyectando una mezcla de 4 ml de contraste y 4 ml de solución salina isotónica a través de la vaina introductora. Luego se obtuvieron imágenes radiográficas en una proyección anteroposterior.

Intervención coronaria transradial

El abordaje estándar fue el acceso radial derecho convencional. Para la coronariografía, se emplearon vainas y catéteres hidrofílicos de 5 Fr. En todos aquellos casos en los que fue necesario revascularizar al paciente, se realizó una intervención coronaria percutánea ad hoc, empleando catéteres guía de 6 Fr tras cambiar la vaina de 5 por una de 6 Fr. La dosis normal de heparina suele ser 5.000 UI (2.500 UI para anticoagulación oral con un antagonista de la vitamina K).

Intervención para lograr la hemostasia

Al final de la intervención, se retiró la vaina introductora y se logró la hemostasia con un dispositivo de compresión hemostática (TR BAND, TERUMO Medical, Japón). Se realizó sistemáticamente una prueba de flujo inverso (Barbeau)5. Las enfermeras retiraron el dispositivo de hemostasia en la unidad de hospitalización. No se siguió ningún protocolo estandarizado para la duración de la hemostasia.

Valoración de la permeabilidad de la arteria radial tras la intervención

Se realizaron ecografías Doppler antes y después de cada intervención transradial. Para valorar la OAR, se empleó el Doppler pulsado en ambas arterias, radial y cubital. Las señales bi o trifásicas confirmaron que el flujo arterial era normal y, por lo tanto, indicativo de una buena perfusión. En caso de OAR, se realizaron otras 2 ecografías al cabo de 1 y 3 meses, siguiendo el mismo protocolo. La permeabilidad de la arteria la valoróun operador independiente.

Clasificaciones y definiciones

La OAR se definió como la ausencia de flujo anterógrado en la AR según la ecografía (figura 1). La ubicación de la oclusión radial se identificó con el Doppler color y pulsado. Se delinearon 3 territorios anatómicos: el tercio distal, desde el estiloides radial hasta unos 7-10 cm proximalmente; el tercio proximal, desde los pliegues del codo hasta unos 7-10 cm distalmente y el tercio medio, ubicado entre las citadas 2 regiones (región media del antebrazo).

Se valoró el tipo de hemostasia, oclusiva o permeable: la segunda quedó confirmada por la presencia de una señal pletismográfica en la AR durante la prueba de Barbeau inversa5 que supone la compresión de la arteria cubital. El operador no intervino durante este proceso, solo dejó constancia de si la arteria se mantuvo permeable o no.

El diámetro luminal interno de la AR se definió como la distancia entre los bordes de la interfaz íntima-lumen de la pared superficial y la interfaz lumen-íntima de la pared profunda6.

La relación R/S (arteria radial/vaina) se calculó dividiendo el diámetro luminal de la AR por el diámetro externo de la vaina introductora (Radiofocus II: 5 Fr = 2,29 mm; 6 Fr = 2,62 mm; 7 Fr = 2,97 mm; Prelude: 5 Fr = 2,52 mm; 6 Fr = 2,83 mm) y se clasificó cualitativamente como < 1 o ≥ 1.

Las variaciones anatómicas de la AR de importancia clínica se clasificaron según las definiciones proporcionadas en la literatura7,8. El origen alto (bifurcación alta) de la AR (figura 2) se definió con relación a la línea intercondílea del húmero. El circuito radiocubital se caracterizó por la presencia de un bucle completo de 360° de la AR y la tortuosidad radial se identificó por curvaturas > 45°.

Se obtuvo un perfil de presión arterial en el mismo lado empleado para el abordaje radial. Los hematomas del antebrazo se clasificaron según los criterios del estudio EASY9: tipo I: < 5 cm de diámetro; tipo II: < 10 cm; tipo III: > 10 cm (sin afectación del codo); tipo IV: afectación más allá del codo; tipo V: con resultado de lesión isquémica.

Análisis estadístico

El análisis estadístico se realizó conel software IBM SPSS versión 25. Los parámetros de interés se acompañan de sus correspondientes intervalos de confianza del 95% (IC95%). Se mantuvo un umbral de significación del 5% en todas las pruebas, que se realizaron bilateralmente. Se llevaron a cabo las siguientes pruebas para realizar comparativas intergrupos: la prueba de la X2 para comparar 2 variables cualitativas, la prueba t de Student o el análisis de varianza para comparar una variable cuantitativa y otra cualitativa y la prueba exacta de Fisher cuando las varianzas eran distintas; también se usó la regresión logística para identificar predictores de OAR.

RESULTADOS

Características clínicas y de la intervención de la población de estudio

Durante el periodo del estudio, se evaluó a 441 pacientes. En 14 de estos pacientes fracasó el abordaje transradial, y se los excluyó del estudio, que contó con una muestra final elegible de 427 pacientes (media de edad: 61,9 ± 11,1 años, el 67,4% varones). Un total de 260 de estos pacientes eran hipertensos (60,9%) y casi la mitad, diabéticos (48,9%).

La tabla 1 resume los datos de la intervención. Las vainas introductoras empleadas fueron, principalmente, de 6 Fr (83,6%) y la heparina se inyectó intraarterialmente en el 63,5% de los pacientes. La dosis media de heparina fue de 5.669 ± 1.394 UI, que se incrementó cuando se realizó una intervención coronaria percutánea (4.940 ± 339 frente a 7.491 ± 1.368 UI; p < 0,001).

Incidencia y características de la oclusión de la arteria radial

La OAR se produjo en 48 pacientes (11,24%), el 89,6% de los cuales fueron asintomáticos, el pulso radial permaneció palpable en 14 pacientes (29,2%). Se perdió a 2 pacientes en el seguimiento a 1 mes. Entre los 46 restantes, la repermeabilización espontánea se produjo en 13 pacientes (28,3%). La tasa de repermeabilización se situó en el 32,6% (15 casos) al cabo de 3 meses.

La ubicación de la OAR fue el tercio distal en 7 pacientes (14,6%), el tercio medio en 21 pacientes (43,8%) y el proximal en 20 (41,7%).

Predictores de la oclusión de la arteria radial

Los pacientes con OAR eran mucho más jóvenes (tabla 2). Los valores medios de presión arterial sistólica del grupo OAR fueron mucho más bajos (138,04 ± 21,92, frente a 145,84 mmHg ± 21,10; p = 0,017). La prueba de Barbeau tipo A se asoció a un mayor riesgo de OAR que los tipos B y C y los pacientes con oclusión presentaban diámetros de la AR más pequeños (2,34 ± 0,40 frente a 2,61 mm ± 0,37; p < 0,001) (figura 3).

Los factores de la intervención asociados a la OAR se enumeran en la tabla 2. Se halló una relación R/S < 1 en 35 pacientes del grupo OAR frente a 153 pacientes del grupo sin OAR (72,9 frente al 40,3%; p < 0,001). La dosis media de heparina fue mucho menor en pacientes con OAR (5.007 ± 1.352 frente a 5.754 ± 1.378 UI; p < 0,001), así como también la dosis ajustada al peso (62,31 ± 17,82 UI/kg frente a 75,73 ± 22,57 UI/kg; p < 0,001). Además, la tasa de OAR se redujo ostensiblemente cuando la dosis de heparina superó las 70 UI/kg.

Cuarenta y dos pacientes del grupo OAR tuvieron hemostasia oclusiva frente a 241 del grupo sin OAR (87,5 frente al 63,5%; p = 0,001). Sorprendentemente, dos tercios de nuestros pacientes (283 [66,3%]) tuvieron hemostasias oclusivas. La duración media de la hemostasia fue mayor en presencia de OAR (5,15 h ± 1,41 frente a 4,29 h ± 1,22; p < 0,001).

En el análisis de regresión logística multivariado (figura 4), los siguientes factores resultaron predictores independientes de OAR: una edad más precoz (odds ratio [OR] = 0,642; IC95%, 0,480-0,858; p = 0,031), valores bajos perioperatorios de presión arterial sistólica (OR = 0,598; IC95%, 0,415-0,862; p = 0,007), prueba de Barbeau tipo A (OR = 0,441; IC95%, 0,198-0,981; p = 0,045), diámetro pequeño de la AR (OR = 0,371; IC95%, 0,323-0,618; p = 0,031), anticoagulación insuficiente (OR = 0,287; IC95%, 0,163-0,505; p < 0,001), hemostasia oclusiva (OR = 0,128; IC95%, 0,047-0,353; p < 0,001) y una duración larga de la hemostasia (OR = 1,786; IC95%, 1,428-2,039; p < 0,001).

Variaciones anatómicas de la arteria radial

El diámetro radial medio fue de 2,58 mm ± 0,39, mayor en varones (2,69 ± 0,37 frente a 2,36 mm ± 0,31; P < 0,001) y menor en pacientes diabéticos (2,53 ± 0,38 frente a 2,64 mm ± 0,38; p = 0,003). El diámetro radial medio fue mucho mayor que el diámetro medio de la arteria cubital (2,58 ± 0,39 frente a 2,22 mm ± 0,43; p < 0,001).

Las variaciones anatómicas radiales afectaron a 63 pacientes (14,8%). La variación más común fue el origen alto de la AR descrito en el 63,5% de los casos (40 pacientes), seguido de la tortuosidad radial en el 28,6% (18 pacientes) y el circuito radiocubital en el 7,9% (5 pacientes). Las variaciones anatómicas fueron más habituales entre mujeres (23 frente al 10,8%; P = 0,001) y pacientes de edad avanzada con una media de edad de 66,3 ± 10,2 frente a 61,2 años ± 11,2 en aquellos sin variaciones (p = 0,001).

Complicaciones perioperatorias

El espasmo radial sobrevino en 72 pacientes (16,9%), una complicación que fue más habitual en mujeres (29 frente al 10,1%; p < 0,001), pacientes diabéticos (22,5 frente al 11,5%; p = 0,002) y cuando se usaron catéteres de 6 Fr (14 frente a 24%; p = 0,035). Los hematomas en la región del antebrazo se describieron en 25 pacientes (5,85%). Según la clasificación EASY9 la mayoría de los hematomas fueron tipo I (17 pacientes, 68%), seguido de tipo II (6 pacientes, 24%) y tipo III (solo 2 pacientes, 8%).

DISCUSIÓN

La tasa de OAR sigue siendo relativamente alta en algunos centros10,11. En el estudio PROPHET, la incidencia aguda de OAR (12%) se redujo casi a la mitad en 28 días (7%)3. La repermeabilización sobreviene como resultado de la activación de la fibrinolisis primaria12. En el presente estudio, la tasa de repermeabilización radial a los 3 meses fue del 32,6%. El único predictor de repermeabilización fue el diámetro radial: a mayor diámetro, mayor tasa de repermeabilización espontánea.

Zankl et al.13 informaron que la OAR se localizó en el tercio distal del antebrazo en el 49% de los pacientes, en el tercio distal y medio en el 13,7% y en todo el antebrazo (tercio proximal) en el 37,3%. Las disecciones de la media también sobrevienen en la AR proximal secundarias, probablemente, al avance o manipulación del catéter sin la protección de la vaina introductora14. En nuestra opinión, esto explicaría la localización de la OAR en la región proximal de la arteria.

El 29,2% de nuestros pacientes con OAR tenía pulso radial. Según Uhlemann et al.4 en el 19,5% de los pacientes con OAR en el Doppler, el pulso radial seguía siendo palpable, quizás por el llenado retrógrado de la AR por las colaterales. En este sentido, el diagnóstico de OAR se debe confirmar con un método más objetivo, como una ecografía Doppler.

Una edad más baja es un predictor de las OAR, probablemente por la mayor reactividad simpática de las personas más jóvenes, lo cual, a su vez, aumenta su riesgo de espasmo. No obstante, esta característica no influye en la tasa de repermeabiliación, quizás porque los espasmos radiales prolongados provocan la formación de trombos intraarteriales permanentes.

Los valores medios bajos de presión arterial sistólica también fueron predictores de la ocurrencia de oclusiones radiales. Se especula con la posibilidad de que tanto la hipertensión como la rigidez arterial pudiesen prevenir la interrupción completa del flujo durante la compresión, contribuyendo así a mantener la permeabilidad radial15.

Se observó una mayor incidencia de las oclusiones con la prueba de Barbeau tipo A. Pensamos que, en aquellos casos con circulación cubital bien desarrollada, la arteria cubital genera un flujo retrógrado competitivo que se opone al flujo radial y fomenta la aparición de oclusiones y dificulta la repermeabilización.

La probabilidad de desarrollar OAR se asocia al tamaño de la vaina introductora16 o más concretamente, a la relación R/S17. Un registro prospectivo confirmó que las vainas de 5 Fr reducían la tasa de OAR hasta en un 55% frente a las de 6 Fr4.

Un estudio de Pancholy et al.18 demostró que la heparina IV es tan efectiva como la intraarterial a la hora de reducir la incidencia de OAR, lo cual sugiere que el efecto sistémico de la heparina es más importante que su efecto local. Un metanálisis publicado recientemente identificó dosis más altas de heparina como la medida más significativa para reducir la OAR12. Este resultado coincide con nuestro hallazgo de que dosis < 70 UI/kg fomentan la aparición de OAR. La alta prevalencia de OAR y el beneficio de dosis más altas de heparina no fraccionada (≥ 50 UI/kg) en este contexto también fueron hallazgos derivados de un metanálisis de 112 estudios19. En un estudio de superioridad aleatorizado que comparó dosis altas (100 UI/kg) y estándar (50 UI/kg) de heparina, la tasa de OAR resultó ser mucho menor en el grupo a tratamien- to con dosis altas20. Evidencias recientes sugieren que una dosis baja de rivaroxabán administrada por vía oral tras una intervención transradial podría minimizar la ocurrencia de OAR al cabo de 1 mes21,22.

Empleando la prueba de flujo inverso (Barbeau), Sanmartin et al.23 hallaron que el 60% de los pacientes no tenía flujo radial durante la compresión, observaciones que dieron lugar al concepto de hemostasia no oclusiva (hemostasia permeable). En el estudio PROPHET3 la OAR fue mucho menos habitual en el grupo tratado de hemostasia no oclusiva que en el de control.

La duración de la compresión hemostática se ha estudiado en estudios grandes y aleatorizados24-26. Los autores concluyeron que la duración de la compresión fue un sólido predictor de la OAR.

En un metanálisis de Rashid et al.27 la incidencia de OAR tras la coronariografía diagnóstica fue mucho más alta que la intervención coronaria percutánea, posiblemente por el uso de dosis más altas de anticoagulación durante las intervenciones12. No obstante, otros estudios han mostrado hallazgos opuestos.

En nuestra muestra, el diámetro radial medio fue de 2,58 ± 0,39 mm y mucho mayor entre los varones. Velasco et al.28 informaron de diámetros arteriales medios de 2,22 ± 0,35 mm y un estudio polaco confirmó diámetros medios de 2,17 ± 0,53 mm para la AR derecha y 2,25 ± 0,43 mm para la izquierda29. Aunque la arteria cubital también se emplea en cardiología intervencionista30 todavía no hay consenso sobre su tamaño frente a la AR.

Las autopsias practicadas sobre variaciones anatómicas arteriales en extremidades superiores confirman frecuencias entre el 4 y el 18,5%8. En la literatura médica, la variación anatómica más habitual de la AR es la bifurcación alta. Yoo et al.31 refieren una incidencia del origen radial alto del 2,4% en 1.191 pacientes coreanos. La tortuosidad de la AR suele afectar a pacientes con origen radial alto, posiblemente por el trayecto elongado de la AR que predispone a la tortuosidad, considerada una de las causas más habituales de fracaso de la intervención, junto al espasmo radial32.

El circuito radiocubital es la causa más habitual de fracaso de la intervención entre hemodinamistas experimentados33. La valoración angiográfica de la anastomosis radiocubital es obligatoria en tales casos ya que, con frecuencia, se suelen observar anastomosis negociables entre las arterias radial y cubital.

En nuestro estudio, el espasmo radial fue la principal causa de fracaso de la intervención ya que sobrevino en el 50% de los 14 pacientes que experimentaron tales fracasos. Un análisis multivariado realizado por Ruiz-Salmerón et al.34 confirmó que las variaciones anatómicas de la AR guardaron una estrecha relación con los espasmos radiales. La relación entre el espasmo radial y las variaciones anatómicas se explica por la fuerte correlación con el origen radial alto y el circuito radiocubital.

Limitaciones del estudio

Como este estudio es un registro prospectivo y no un ensayo aleatorizado, no se puede excluir el sesgo de selección. Nuestro estudio es representativo de una experiencia unicéntrica con un número limitado de pacientes, a pesar de ser uno de los registros prospectivos más grandes de ecografía vascular en cateterismo radial realizados hasta la fecha. Entre otras limitaciones del estudio, señalamos la falta de protocolos estandarizados para el uso de heparina y técnicas de compresión.

CONCLUSIONES

Ante el número cada vez mayor de intervenciones transradiales que se realizan y la mayor edad de los pacientes tratados con estas intervenciones que complica, más si cabe, estas intervenciones, resulta esencial mantener la permeabilidad de la AR para futuros accesos. Aunque se han identificado predictores de OAR tras el cateterismo cardiaco, en la práctica implementar medidas preventivas sigue siendo todo un desafío. Los principales predictores modificables asociados al riesgo de OAR son una heparinización insuficiente y la presencia de hemostasia oclusiva. En este sentido, las estrategias preventivas deberían de centrarse, ante todo, en abordar estas 2 cuestiones.

  • A pesar de las recomendaciones en materia de prevención de OAR en cardiología intervencionista, su incidencia sigue siendo relativamente alta en algunos centros.
  • La repermeabilización de la arteria puede sobrevenir espontáneamente durante el seguimiento.
  • La oclusión permanente de la arteria radial impide cualquier posible uso en el futuro (procedimientos intervencionistas, diálisis, etc.)

¿QUÉ APORTA DE NUEVO?

  • La OAR no se limita a la región distal de la arteria pudiendo haber afectación del vaso en toda su longitud.
  • El diagnóstico de OAR debe confirmarse mediante ecografía Doppler, que sigue siendo el patrón oro.
  • Los 2 predictores independientes modificables de OAR son el protocolo de anticoagulación y la técnica de hemostasia.
  • Las variaciones anatómicas de la AR pueden influir en la intervención. El origen alto de la AR es el más habitual seguido de las tortuosidades radiales. Tras el espasmo radial, el circuito radiocubital es la causa más habitual de fracaso de la intervención con hemodinamistas experimentados.
